PLANNING FOR RETIREMENT!

Employees that plan to retire who are over age 65 and do not have or have not applied to Medicare Part B need to do the following. There is a form that will need to be completed and signed by the company prior to leaving employment. The form “Request for Employment Information” and “Application for Medicare Part B” can be found in the file download section of this site. After this form is filled out and signed by someone in HR, confirming that the employee has been covered by medical and prescription coverage, the employee will take the ”Request for Employment Information” form and “Application for Medicare Part B” form to the Social Security office for processing. This process should be started approximately 90 days before leaving employment.
